Please read these Terms carefully before using ElevateMD’s website or services. By accessing or using our platform, you agree to be bound by these Terms.

1. Acceptance of terms
By accessing, browsing, or using the ElevateMD website, patient portal, telehealth platform, or any related services (the “Services”), you acknowledge that you have read, understood, and agree to be bound by these Terms of Service (“Terms”). These Terms constitute a legally binding agreement between you and RegenHairSolutions LLC dba ElevateMD (“ElevateMD,” “we,” “us,” or “our”).
If you do not agree to these Terms, you must not use the Services. Your continued use following any changes constitutes acceptance of those changes. You represent that you are at least 18 years of age and have the legal capacity to enter into this agreement.
2. Description of services
ElevateMD provides physician-directed telehealth services focused on longevity medicine and wellness therapies for patients located in states where ElevateMD physicians are licensed (currently Florida, Texas, Alabama, Ohio, Michigan, Maryland, Washington, D.C., Pennsylvania, New Jersey, Delaware). Our Services include physician consultations via telehealth, clinical evaluation and medical-history review, prescription recommendations based on physician assessment, ongoing monitoring, at-home laboratory coordination, and educational content.
All medications prescribed through ElevateMD are compounded by independently licensed 503A pharmacies under physician prescription. ElevateMD does not guarantee any specific medical outcome. All treatment plans are individualized and subject to physician approval based on clinical evaluation, and are available exclusively to patients located in a licensed state at the time of consultation.
3. User accounts
To access certain features you may need to create an account through our patient portal. You agree to:
- Provide accurate, current, and complete information during registration and in all medical questionnaires
- Maintain and promptly update your account information
- Keep your account credentials confidential
- Accept responsibility for all activity under your account
- Notify ElevateMD immediately of any unauthorized use
ElevateMD reserves the right to suspend or terminate your account if we reasonably believe you have violated these Terms, provided inaccurate medical information, or engaged in fraudulent activity. You may not transfer or share your account.
4. Telehealth consent and limitations
By using the Services, you acknowledge and consent to the following:
- Telehealth delivers healthcare using electronic communications between a physician and a patient who are not in the same location
- Transmitted information may include medical history, findings, laboratory results, and treatment recommendations
- Telehealth has inherent limitations, including the inability to perform a physical examination; your physician determines whether it is appropriate for you
- Technical difficulties may interrupt the delivery of telehealth services
- All telehealth interactions are documented in your medical record and protected under applicable privacy laws, including HIPAA
Emergency disclaimer: ElevateMD services are NOT for medical emergencies. If you are experiencing an emergency, call 911 or go to your nearest emergency room. You may refuse or withdraw consent to telehealth at any time. ElevateMD does not prescribe controlled substances during initial consultations and may decline to prescribe any medication at the physician’s sole clinical discretion.
5. SMS communications (A2P 10DLC)
By providing your mobile number and opting in, you consent to receive SMS/MMS messages such as appointment reminders, treatment and care-coordination updates, lab notifications, prescription and shipment updates, post-treatment instructions, educational content, account and billing notices, reorder reminders, and service announcements. Message frequency varies (typically 1–20+ per month).
Age: You must be 18+ to opt in. Opt-out: reply STOP, UNSUBSCRIBE, CANCEL, END, or QUIT to any message; or email [email protected]; or call (786) 574-2428. Opting out will not affect the delivery of your medical services. Rates: standard message and data rates may apply; ElevateMD is not responsible for carrier charges. Carriers are not liable for delayed or undelivered messages. Your consent to receive SMS is not a condition of purchase.
AI-Assisted Communications. Some ElevateMD communications (such as SMS and social messaging) may be generated or assisted by an automated AI system. AI-assisted messages are limited to general, administrative, and product information and do not constitute medical advice. Any clinical question is routed to ElevateMD’s licensed clinical team. You can reach a human at any time by replying HUMAN or STOP.
6. Payment and billing
All pricing is displayed on our website and communicated prior to purchase. By purchasing, you agree:
- Cash-pay: ElevateMD does not accept insurance; you are responsible for the full cost at purchase or per your subscription terms
- Transparent pricing: all fees are clearly displayed, with no hidden surcharges
- Auto-renewal: subscription services automatically renew at the end of each billing cycle unless cancelled beforehand via the portal, email, or phone
- Refunds are governed by our Refund Policy, incorporated by reference
- Late payments may result in suspension, account holds, or collections, with late fees to the extent permitted by law
7. Intellectual property
All content, trademarks, logos, text, graphics, images, software, and other intellectual property on the ElevateMD website and patient portal are the exclusive property of RegenHairSolutions LLC dba ElevateMD and are protected by applicable laws. You may not reproduce, distribute, modify, or republish any content without prior written permission, except for incidental browser caching and printing or downloading one copy of a reasonable number of pages for personal, non-commercial use.
8. Limitation of liability
Disclaimer of warranties: The Services are provided “as is” and “as available” without warranties of any kind. ElevateMD does not warrant that the Services will be uninterrupted, timely, secure, or error-free.
Medical disclaimer: Information provided through the Services is for informational purposes only and does not constitute medical advice. All treatment recommendations are made by licensed physicians based on individual evaluation. ElevateMD does not guarantee any specific health outcome, and individual results may vary.
Limitation of damages: To the fullest extent permitted by law, RegenHairSolutions LLC dba ElevateMD and its officers, directors, employees, agents, and affiliates shall not be liable for any indirect, incidental, special, consequential, or punitive damages. Aggregate liability for all claims shall not exceed the total amount you paid to ElevateMD during the twelve (12) months preceding the event giving rise to the claim.
9. Governing law
These Terms are governed by the laws of the State of Florida, without regard to conflict-of-law principles. Any action arising out of these Terms or the Services shall be instituted exclusively in the state or federal courts located in Sarasota County, Florida, and you consent to the personal jurisdiction of such courts. Any claim must be filed within one (1) year after it arose, or it is forever barred.
10. Changes to terms
ElevateMD may modify these Terms at any time. Changes are effective upon posting the revised Terms on the ElevateMD website, and the “Last updated” date will be revised accordingly. Your continued use following revised Terms constitutes acceptance.
